Let's test that.
The bell is for Discussions and Announcements, the envelope is for Forum and Message Wall. This should give you one on the envelope.
Do you have this on any site? Do you have any personal js or css? What are your email settings in your preferences?
I was mistaken, I was getting emails, I was just looking in the wrong folder on my email. However, not getting bell icon notifications, but am getting message notifications.

At the very least, try this next time you think you have messages. If it works, vary with the commands until you've found a culprit. If it doesn't... send a bug report with this form.
This is with the Red Dead Wiki, and I have used the GTA wiki, had not visited the site in a while (more than a year), and never got any notifications about followed pages there.

This might seem a like stupid thing to have to clarify. However, I am going to do it just on the off chance that this is actually the issue. The notification menu does not contain a one-for-one of your email. At the moment, the notification menu only works with Announcements and Discussions. It does not contain notifications for pages you are following, for example.
To be even more clear: the bell icon is for Announcements and Discussions replies/likes and the envelope icon is for Forum and Blog comments replies/kudos.
The envelope icon disappears when you're in Discussions.

Yeah not kudos, and only if you are the author of the blog.
