How do I make boxes like these in a Wiki

I want to make a template for a Wiki I edit, but I can't seem to recreate this box. Does anyone know if I have to ask the admins to download a script for it?

I tried to copy and paste (for your information, I'm editing the official brazilian version of the site) but the box doesn't appear for some reason. Is it a plugin I have to install?
