That is interesting. I don't know what program you are using but here is what is happening for me. When I view the original image as uploaded in your opening post, my browser says and displays it as 256x256. When I download it and check the file properties, it says 128x128. However, if I open the image in a program, even a "simple" one like MS Paint, it says 256x256. I don't know what is going on but something is off with the way the program you are using writes its files. Even if it is within the size limits, the mismatched data might still trigger an error.

Huh. I have never used that before. Either way, I think I have figured out what is going on. I didn't know this before but apparently ICO files are not single-image files. You can store multiple images in a single ICO files to provide the icon at various sizes. When I open the ICO file using a more advanced image editing program, it reveals that there are 9 different images in the file; each corresponding to a different icon size. They are 16, 24, 32, 48, 64, 72, 96, 128, and 256. Depending on what Fandom's software is configured to take, this might be an issue as well.
I looked up some instructions online and they say that MS Paint 3D cannot save ICO files. Can you confirm this? If so, what have you been using to convert the image to an ICO file? The extra sizes might be added by the converter.

Damn, oasis skin never gave a problem regarding it. Just resize the image, convert it and there you go.

Get an original .ico instead of using png/jpeg to ico converter for the icon.

No, if you get a bad ico, try a different converter. Unfortunately, I forget which one is which.

Adding on to what Fandyllic said, try using a converter that makes multiple versions of the icon. At least one of the results should work fine with Theme Designer.
Yeah, there definitely should be converters out there that don't cause this issue. So just try a different one. By the way, which converter are you using?
