1. A few things to take note of:
   • Different browsers support different file types. What works for one browser may not work for another.
   • The image must be small enough. If the image is too big, browsers tend to just ignore it.
   • Always provide a standard fallback option. If the computer/browser is not able to use your specified image, it will use the fallback instead. Without a fallback, you run the risk of the cursor being invisible.

3. You could use DPL similar to this markup for random images. However, you won't be able to distinguish between uploading new versions and just editing the file page. As such, you need to decide what you mean by "new". Assuming you only care about when the first version was uploaded, you could use something like this:
