As the template is right now, it defaults to the second form as seen here. The problem is that for one of the pages it needs to default to the first form.
Currently yes, but my fellow administrator may have his own plans which would require being able to set defaults.
Ok, well I'm not very familiar with toggles, but I'll fiddle around with it and see if I can't come up with something. In the meantime, anyone who knows more about this is free to try it.

I managed to get it mostly working. You can test it out in my sandbox if you want. It defaults to the first form when the "test" parameter is 1. If you change that to 2, it defaults to the second form. The only problem with it is that it doesn't show up at all if test is missing. I won't have time to fix that today, but if you don't mind waiting until tomorrow, I should have it done by then.
